My advice would be to go to court and get a court appointed power of attorney, especially if he is declining. Also, you need to be honest with yourself. I understand he wants to die at home (we all do) but is that what is truly best for him? At some point his condition will overwhelm you and your family's abilities. By then it may be too late. Are you sure that no SSN was ever issued for you? Since 1989 SSA has had a program called Enumeration at Birth. Most SSN's are issued to new born babies based on information supplied by the state registrar when the child is born. You do need your own ID Information--your name, date of birth, place of birth and parents' names including mother's maiden name. I suggest you get a long form birth certificate which contains all of this information. The long form birth certificate may also show your SSN.
