Cost of living trust?

I know how to change your name through one of the various online and offline deed poll people, but I had a friend do this, and all they got was a piece of paper that was (basically) a document saying "X is no longer my name, it is now Y". Then she had to sign at the bottom, and one witness had to sign.. . At no point was there anything official (as in, there was nothing recorded at the government or anything)... so my question is, do you even need to pay anyone for this bit of paper? Or can you just write it out yourself and get a witness to sign it?. . As paying up to ?40 to some companies (most are about ?7 to ?10) seems a little pointless if all it is is a piece of paper (a contract basically)!. . Just wondered if anyone had a proper answer to this?.
