Got divorced in February of this year and want to change back to my maiden name officially. How do I go about and what cost am I looking at?.
I can see that you would want some kind of paper that the other person signs that says they received their money but it doesn't work like that in this kind of situation. The legal way is to either write them a check or give them a money order for the amount owed, but their name needs to be on that document (so when they cash that well then that's your proof that it's been repaid). The rough part is that that person might be hesitant to accept a paper type payment. If he does though is you'd have a receipt of that transaction.. . A printable document isn't going to help you as more than likely they won't sign it. What was the money for?
How do I create an order form in Microsoft Access?
I will be taking care of a friend's child in my home while they work. We would like to have a contract and any other helpful paperwork to make it more official and protect our friendship. Any advice on what to include in the contract (in regards to sick time, vacations, and overtime)? I have children of my own, so I was wondering how it works if one of my kids is sick or if we go on a family vacation. I would love to find a contract template that I could customize for our personal requirements. Also, does anyone have any ideas on how to keep track of receipts/payment/etc for tax purposes?. Thank you!.

A form used for official purposes usually requires an official "letterhead" on the top, with an exactly reproduced (not just similar) identification seal. In government, that would be at least the exact agency name, and usually the name of the agency head
