it really depends on the reason why you're changing your name and where you live but from where I live . a parent needs to fill out a consent form, and those older than 12 has to fill one out too, and you pay a certain amount of money to change, then you'd have to change everything like birth certificates, passport, school board etc and then after for some finishing you'd need to do something with the courts
