my husband changed his last name years ago. his new last name is recognized on his ss card, on his military records and other documents. we recently moved to Georgia and he has to get a ga license. but his birth certificate still has his old last name. we can't seem to find official state papers showing the name change. he was born in Maryland, changed his name in North Carolina. Where should we look for those documents? What department? phone numbers or web sites would really help. Thanks..

my professor has asked me to "send him some info to put into the letter." I am asking him to write me a general reference letter which I plan to use to apply to a volunteering position at a hospital. I've taken two courses with him, had great attendance and participation, and got A's in both the courses, so what should I ask him to write?? the two courses are sociology courses btw (sociology of aging and sociology of health).
