for example, when you apply for a job with exam certificates under your old name, or when updating your passport.. . It's advisable to have a legal professional help you change your name, but you don't need to. You can change your name yourself free of charge. You can go to a stationer's shop like WHSmiths and buy a kit containing blank documents to fill in and instructions. Alternatively, simply make a document yourself that reads like this:. . Change of name deed by [former name] of [address]. . I have given up my name [former name] and have adopted for all purposes the name [new name].. . Signed as a deed on [date] as [former name] and [new name] in the presence of [witness's name, signature and address].. . And sign it with a witness. You should make copies and leave them somewhere safe because you may need to present the document in the future.. . If you do this, you have legally changed your name. However, there's still more to do.. . Next, you'll need to have your name changed on bank accounts, official documents, NHS records etc. This is the most time-consuming and annoying phase of changing your name. You'll need to write all the organisations with records that need changing, as well as to the DVLA, passport office, etc. They'll need copy of your Deed of Change of Name. Sometimes, you need to pay an administrative fee. New drivers' licences and passports also cost money and these will need to be issued.. .
