How can I pass the authority to sign up a housing title form to another person?

does my living will/advanced healthcare directive remain in effect regardless of location and/or citizenship, or do I need to write a new living will? Please note that my current living will includes the clause "This is the last iteration of this living will/advanced healthcare directive, and it overrides and any all documents signed by me at any date, past present, or future." Would this clause only apply in California to me as a US citizen, or does it apply anywhere in the world?
