I don't know what type of insurance you have at work but I have never heard of one letting you cover your grandparents, only spouses and children.
formal letter writing?
When your parents legally adopted your child, did they also fight to have the birth certificate changed? I doubt they did that. Just because your parents adopted your child does not mean the birth certificate was changed, and I would be shocked if they did that. Contact your local county offices to determine if indeed their names are even listed on the birth certificate as parents. If they are, you should be able to prove that you are the birth mother and have re-adopted your child to get it changed, but I'm guessing it won't be free.
It is very simple to dispute errors on your credit reports. Use the online dispute form or send a letter indicating the error.. . However, if you are trying to remove legitimate derogatory items, save the postage. Even if something does fall thru the cracks and come off, it is very likely to return when the creditor next updates. There are no special form letters or magic wands.
