SAMPLE COVER LETTER NEEDED!!!!?

sample of business letter?

I had a child with a man in West Virginia, and left him when she was 6 months old. We were never wed, but his name is on the birth certificate and her last name is his.. . She is now 4 and a quarter old, and I have heard rumors of being able to change her last name without his consent to my maiden name.. . He lives over 12 hours away, does not pay child support, does not see her, does not wish to see her, and has not been a part of her life since she was 6 months old.. . I now live in Florida. What steps do I take to change her last name, and is there some sort of 'short cut', or do I need to do the entire shebang within the legal system of name changes with parental consent?. . She is nearing pre-school and I'd like to not begin the confusion of differing last names.

Ok so i work for a fashion retail shop. only been there 2 weeks. they only give me 4 hrs a WEEK! my managered messaged me that my hours are cut and not to come in until later tomorrow.

I live in Seattle, so the natural choices seem to be Bank of America, Washington Mutual, or US Bank.
