The doctor is giving her 30 days to find a new physician and that he will be available for any emergencies during those 30 days. After choosing another physician, mention that a copy of her medical records will be transferred to the new physician of her choice upon her notification. Conclude the letter by thanking the patient for having choice the practice for her care.. . I also need to create a name of an office / phone # / fax # / doctor name and last name.

My question is this: Do I need to add, can I add, and what wording can I use to add... What happens to the photographs should full payment not be received after a certain period? Also how long should that period be? (and yes I know full payment should be made before the event date). . Basically, I covered an event for someone (no contract) and the arrangement (as I trusted this person) was that I would let them know when the photographs were ready and they could pay then (mistake I know and wont be making again). After numerous attempts to arrange for me to get my payment and them to get their photos any mention of the photographs were either ignored completely or the subject was changed. This was over a year ago and although I know they no longer have the interest for the actual photographs or the intention of paying, I'm in two minds about what to do with the images, so advice on this also would also be great. This is the only one that has caused me any problems.. Just to clarify wedding and events will be two separate contracts. In response to "matt" answer - With or without a contract do I have a right to allow the guests (some of who I know) to purchase the images that they are in?.
