does a power of attorney give the right to make healthcare decisions when the person in question is competent?

I really do not know where to start at all, what to include etc because i have wrote a lot on my CV.. I am applying for abode hotel in Chester which opens in April and they are accepting CVs and Cover letters now through email. The position i am applying for is 'bar staff'. . You should get evidence supporting your name change, however, such as advertising in a newspaper, preparing a statutory declaration, or getting a deed poll. A deed poll shouldn't be needed for most purposes, aside from getting a passport.

Someone used my name and social security number to open several accounts without my permission or knowledge. I have resolved this matter with all of the companies, but only one, Verizon, will not dispute the account. They refuse to accept "Identity Theft" as an explaination and continue to report the account as accurate, despite the stack of documents I have sent them to prove otherwise. I need a lawyer to help me with this, but I don't know what type of lawyer I need. I'm in Georgia if anyone can refer someone..
