Ironically, even with a Power of Attorney (Durable) or Guardian-ad-lidem, in some states, a person who is deemed a danger to themselves or others, deemed physically or mentally incapacitated, etc can still refuse medication and treatment. My mother is going through a situation currently in Arizona, as her brother-in-laws POA (Durable) that has caused us to really have to brush up on our legal education. Some of the laws are just ridiculous. . From my understanding, any person has the right to refuse medical treatment. Once deemed a threat to themselves or others, you can petition for an involuntary committment, but the guidelines for a committment are more strict than one would hope for.

Yes.. . You can change it to any name you want.. . His last name.. . Your last name.. . A combination of the two names.. . or ?. . A name you make up.. . See the link below.. . Peace.

mine aren't more than 10 sentences or so.. let me google for an example.. brb. Also, there are usually examples of them on Word and other programs..

So which one is the correct spelling? No you can't just call social security and tell them your story and they will send you your new social security card. If the name on your card is incorrect then you need to take your birth certificate and proof of ID to the office and apply for a replacement card. ID can be a school record showing name and date of birth or age or any other record which gives that information. If the name is misspelled on your passport then you should be correcting the passport.

You can change your name to whatever you want - whether you're married or not. All you have to do is file a petition for a name change with the courts. It is a tradition that when a woman gets married she takes on her husband's last name, but it's not a requirement - you don't have to change your name at all. Some women keep their own name, some hyphenate, some keep their maiden names as middle names and take on their husband's last name (which is what you're thinking of doing, sounds like). You need to check the specific rules of the name change in Canada, but the general answer is yes, you can change your name in any manner you want.
