Usually you have to be 18 or have parental permission. You just need to got to your local courthouse and ask for a name change form. Fill it out and they'll file the paperwork. You usually have to explain why you want your name changed, so the judge can approve the circumstances. I've never heard of the judge disapproving a name change, so I wouldn't worry. I think the fees are more like $25-50 rather than $100 like the first poster commented. You'll get your documents in the mail about 2-3 weeks later and your name will be changed on official records at the courthouse.
You can get generic forms on line and such, but that's not something you want to do generically, if it's done wrong you're opening up a big old can o' worms!
