First of all, make a photocpy of the form! They can and do get lost on occasion. Photocopy in a folder, you are ready to go. You need to meet with the teacher, asking if he/she will be able to see you for a couple of minutes before school, after school, or during the teacher's prep period. (Don't ask to see a teacher during lunch -- it's literally the ONLY time they really have for their sanity-break!) If the teacher says yes and agrees to a time and place, that's great -- ask at the time and place agreed, and BE ON TIME! Usually, though a teacher will ask what it's about, and you can make your request right then. A polite request is the way to go, such as "As you may have heard, I'm applying to college, and XYZ College has sent me a form that has to be submitted by one of my teachers. Since I have been in your XYZ class I am hoping that you would be willing to help me." Assuming the teacher says yes, give him/her the form, the return envelope with a stamp already on it, and say thank you. Some teachers will show you what they are sending and some will not. Personally, when I was teaching I ALWAYS gave a photocopy to the student. In the rare event a teacher says no, don't take it personally. Ask another teacher, and if you get two refusals see the school Principal and explain the situation. This is not nearly as difficult as you think -- just scary! Most teachers are flattered to be asked and will say nice things. Just don't interrupt a class or their lunch hour.

If your child is traveling with one parent: The letter needs to be signed by the parent not on the trip. I recommend you take two originals in the event immigration authorities need to keep one on entry at your destination. You will need a valid ID for the notary. The documents must be signed at the time of notarization, not before. Be sure to include a copy of the child's proof of medical insurance coverage. It is recommended that divorced parents take a copy of their custody agreement as well.

From what I know, a contract or agreement is legal as long as both parties agree to terms and conditions verbally or written. If that's the case, why do professional contracts include strange words such as: Witnesseth, Whereas, Hereto...etc..? Are these words necessary in a written contract? If I were to make a simple agreement for me and a friend (over a matter of $500-$1000) and we both agreed and signed, would that hold up in court?.
