The last 3 are correct as you have them.. . The first though is questionable - there are many templates available online to help you create an organization chart. There would be no problem taking the format the company has used as there are only so many of these chart styles available to the numerous business types.. . If that style suits your organization then there is no reason not to use it. However if you just copied it exactly as it is with the same heirarchy then you may find that it has more (or less) levels than you require.. . You could probably find two organizations that have no connections and have the same organization chart. It does not mean that they have copied each other and it is not a violation of business or computer ethics.
