You need to send the revocation to whomever you have filed the original POA with. Link below to free form that you can copy into your word processing package and complete.. . If you originally received the POA through an attorney, it would probably be good money to go through them to revoke it as well.. . You will need to have your signature notarized to make it binding. You can probably go to your financial institution and have them do this for you.. . Good luck.

My baby sister kicked her husband to the curb six or seven years ago because he was a lying cheat. She never divorced him though because of property issues. Now she is dying of cancer and two weeks ago he showed up and now no one can talk to her unless they go through him. Yesterday she was rushed to the hospital and is in ICU. I made arrangements to fly there (Puerto Rico) to see her and texted him to find out what hospital. He refused to tell me, told me if I came he would not allow me to see her, and made threats towards me when I told him I was coming anyway. There are over 50 hospitals in Puerto Rico. I can't afford to go down there and spend day after day going from hospital to hospital looking for her, and she doesn't have much time. I've contacted various governmental agencies and everyone tells me I need to come down there. I am broke, and I mean BROKE. He says she doesn't want to see me or any of the family, this is horse crap - I know her and I know she would want her family to be there. Any suggestions?.

All your cover letters should be addressed to her, but each one should be different and SPECIFIC for the job that you're applying for. You should never send the same cover letter out -- period.
