Both me and my wife have passports issued before marriage and of different addresses. Now we want to get surname change for my wife and my name endorsed as spouse besides changing her address. In my case, i want to include her name in spouse and get my own address changed. Delhi Passport website is more confusing than helpful. Does she have to submit Form 1 for the same? and if I give the copy of my passport(with old address) along with joint marriage affidavit and address proof of her bank account(ICICI) will that be enough to get the changes made? If yes how much time will it take. In my case should I just give a marriage affidavit along with an address proof and filled up Form 2. Will these documents be enough?.

Legally speaking, if he is of sound mind he has the right to determine whether he goes into a home or not. If he has dementia or Alzheimers then he can't make the decision. The decision would fall to either his spouse or next of kin.

free templates or forms for nonprofits?

I'm in, for whatever they need or want. I think everyone should be part of the organ registry--you could save up to 5-6 lives.. . Even if you can't donate (Debra you are feeling lots better), you can donate to a medical school and your body could help someone find a cure for your illness.

At several of the colleges I'm applying to, they have a January 1st deadline for the application (and I'm using the Common Application). Since schools are out for winter break, I can't contact teachers for recommendation forms until after January 1st, meaning the recommendations would have to be sent in after the deadline. So when it says the deadline is January 1st, does it mean just the application, or also the recommendations? I would think a student couldn't control whether or not a teacher got his or her recommendation form in on time..

You might want to check with an attorney....a lot of them have free consultations and some might even take your case on pro bono (free to you), but you just need to check around.
