Help~~~~~~For correcting my English grammar~Thank you!?

My Girlfriend has a mental illness and wants to appoint me Durable Power of Attorney. We just moved to Louisiana from Texas about a month ago. I am typing up the Durable Power of Attorney form, but when it asks me her residence, I do not know what to put, she is on SSI, food stamps and medicaid all through Texas. But at the detention center she is at they have her address as in Louisiana, which is correct, but we just haven't had time to switch everything over yet. Do I type up a Texas Durable Power of Attorney or a Louisiana one. I am thinking Texas because all of her government assistance is still over there. So my questions are, do I put the address on her license which is Texas or the one they have at the detention center which is Louisiana. Second question, if I put a Louisiana residence, then would I have to do a Louisiana Durable Power of Attorney? And one more thing, if I do put Texas, when it ask for City and County, do I put where the detention center is or what her license says? I do not know what to do. Someone please help..

How do I format the return address?. How do I format the recipient's address? Some sources say that it's the opposite of what we do in the US, you start with the country on the first line, then the city, etc. and the recipient's name is the last thing. But other sources say the name goes on the first line and then the house number and the street etc and the country is the last thing. Which is it??. And do I write in English, or in Russian? Or both?. Please give examples!!!!.
