power of attorney - how much?

Last month i had an car accident. my car is totaled, and a friend of mine in the back seat without seat belt smash his face against the chair which lead to surgery (note: he is international student). The surgery went well and my insurance paid for his surgery . So me and him have a agreement that for whatever happen from now on will have no things relate to me. That means in the future he could not sue me because of some weird make up pain. That is why i want to put that agreement on paper and notary it. But i don't know how to get it done. any info is a big help..

Question 1: I went to a notary to have my will signed and notarized. I'm a California resident, so I need at least two and at most three witnesses, so I asked three random people at the notary's office to witness for me, and all of them filled out the proper form and signed. Because I didn't know the witnesses, if there was an issue such as them not using their real names or something like that, could there be a problem regarding the validity of my will? (One girl who witnessed originally had a problem remembering her last name, and no one is that stupid.). . Second question: regarding power of attorney, I don't know anyone who I trust enough to sign over my healthcare and what not if I am incapacitated, but if I'm incapacitated I want them to pull the plug and not do anything to my body mainly for religious reasons--if I sign power of attorney over to someone, I wouldn't want them to authorize any surgeries or anything on my body. Is there some kind of document that I can have on file to mandate that no one can approach my body if I am incapacitated no matter what? (A 'do not resuscitate order' wouldn't be all-encompassing enough, but something like that I suppose.).

I trust this arrangement will be acceptable to you and I await your reply. I would ask that as a result of this offer you now forestall any diligence against me and allow me the opportunity to begin to repay this debt.

I have Microsoft Office 2007 and have a lot of letters to do. I am so fed up with having to type out my address each time. I have tried to set my address as a template but failed miserably, does anyone know if I can do it and if so how? - thanks.
