I'm revising our shelter's off site adoptions paper work and thought it would be neat to attach a picture of a dog or cat on a mailing label to put on the envelope containing the information packet.

The specifics of the legal "stuff" will vary depending on the jurisdiction where you are shooting, possibly on local regulations, the age-of-majority (legal adult), maybe even the citizenship of the subjects. You did not specify where you are but I will guess "somewhere in Allegheny Co.". That warning delivered, here is my Answer.. . The form you are looking for is technically referred to as a "Release". . Before download the freely available template for that form, . you need to "Google" the phrases "model release" and "model release contract law", . and other related terms you discover you read-up and study the information. . Be careful to verify that the information and the forms are specific to your location, . the type of "work" to are shooting, and ALL jurisdictional regulations.

I changed my last name a long time ago. You can check with your local government on when you are legally able to do this, I think it changes from state to state. Normally it is 18 but, you can pick out another name anytime and go by it, a lot of people have aliases. It makes me sad when I hear some one say "I hate my name" as I know how they feel and that is why I changed mine. Legally I changed it by going to the university law library and bought a 'name change' packet for three dollars and when I had all the forms filled out I went to the courthouse and paid a $120 filing fee and they send me to one of the court rooms where it took only 10 minutes for the judge to degree it and stamp my papers.. I loved changing my name on my drivers license, social security card, etc. Good luck.
