I want to change my last name to my mother's maiden name. I am 21 and live in Illinois. I do not have anything to with my dad and I do not get along with my step dad either. What do I need to do? Are there different costs for different counties? What forms do I need and other steps to take? Some people have told me I need a court order. Will I need one, even though I'm 21 or is there a way to change my name without it? My moms side have told me they will help out money wise but I have to get all the information myself.
