Also, where can I get an affidavit to revoke an existing POA that my Mother wishes to change to me? Is there anything else I need to know? Does it require a notary in PA? My Mom is moving home with me for hospice care at age 81 from a nursing home, and that is her LUCID wish. My two older brothers, who currently have financial POA, along with the nursing home and medical staff, are abusing their authority to try to block her from leaving. She wants to make me POA for both healthcare and financial POA. I don't have much time to get these in place because she's coming home Friday. I would appreciate any help you could give me please to streamline my research, and I am on a more than tight budget until her Social Security money starts coming into our household..

5. TERMINATION: This agreement may be terminated earlier upon (1) death of Employee or illness or incapacity that prevents Employee from performing his/her duties for a period of more than _____ weeks in any calendar year and (2) breach of the agreement by Employee.
