My husband and I went through this with our first son. We live in MD, so I'm not sure what the process is in other states, but we contacted our county court and asked them what they needed. We had to first publish our intent to change our son's name in the paper and provide a copy of the article and the paid receipt to the court. We also had various other papers to fill out. After the article runs there is a waiting period, to see if anyone opposes and so forth. Then the court provides documentation that the name change is approved and you then take that court document to the Hall of Records, Social Security, ect... I need to add my dad's last name to my name. I'm a naturalized citizen (living in Illinois) and wasn't aware that I could change my name at the time I recieved my citizenship. Ideally, i would want my current last name as my middle name and my dad's last name as my new last name. Do I still have go to my county court and petition for a name change eventhough I am just adding a last and not really change my name completely? What is the procedure?.
