    It's not really clear what you're asking for, but I'll take a guess.. . You're wanting to be able to e-mail your customers a form that they will fill out and send back to you and it will automatically be plugged into your database?. . I believe Adobe Professional has the function to create forms that folks fill out that can collect and compile all the information.. . I think what you really want though is a web page that people could go to to fill in this info. When web pages collect info, they collect it in a database. You could then connect that database to your database and import the information. You could use a survey maker website to create a "survey" and the survey questions would be the info you want to collect from your customers.. . Oh wait... I just realized that you're asking for a template to e-mail out to folks that would just talk about your intent to collect this info. I think the first two options I suggested are better choices.

    The reason I am considering ending the agreement a bit early is because right now, I am babysitting from 8:30 until about 14:00. My husband was orginally on day's when I agreed to this. He's a member of the USAF and if any of you are familiar with their schedules, they change relatively often. He was on days to start with, working 7:00-16:00. This worked out pretty well because I was up shortly after he left for work, I babysat while he was at work, had lunch ready when he came home for his break, and had the child gone, and house cleaned by 16:30 when he got home. My daughter would go to bed around 21:00 and I would go to bed around 23:00. Get up and repeat the next day. Worked well for our schedules. Then he went to swings, which is 15:30-23:00. This a bit more difficult, but not horrible. Sometimes I'd get less sleep that needed from trying to spend time with my husband when he got off late at night, but I mean that was my fault ya know? No big deal. My husband would go to bed around 2:00 or 3:00, and get up around lunch time. Occasionally the baby would wake him up, but he's a pretty sound sleeper once he's sleeping so very rarely. However, now he has moved to mids, and goes in at 23:00 and gets off at 7:00. He'd like to go to bed around 9:00 or 10:00, but with 2 kids running around, and 10:30 being the child I babysit's naptime, its kinda hard for him to sleep. The child I babysit is a bit of a cryer when you lay him down for naps, He doesn't cry long, but a good 10 minutes or so is quite common for him. And my daughter's room and our room are adjacent to eachother, so its inevitable that my husband is going to hear him crying. He had a LOT of trouble getting to sleep today with the kids, and told me "If this doesn't get any better, then you're going to have to tell her you can't babysit anymore". I really hate to do that, but I have to worry about the functionality of my family first and foremost. Since this is his first week on mids, we're going to wait a few days and see if the situation gets any better, but if not, and I have to stop babysitting, how should I go about telling the mother? She's a really sweet person and so is her son, and I hate to end my agreement early. I've been babysitting him for 4 months, and am suppose to continue babysitting him for 2 more months. In the past when my husband worked nights and slept during the day, my daughter and I would usually get out of the house for a couple hours to let him get to sleep, because I know its hard enough trying to adjust your body to sleeping during the day with the light beaming in and all, but with commotion going on in the house it makes it that much harder.
