Can someone please help me with information on retaining Power of Attorney for a terminally parent?

I am looking for some form building software for my new business. This is a physical business without a web presence, so I need to be able to create forms/surveys/applications/etc for said business - that are printable.

I am going to the store in a little while to get some supplies to start my scrap booking, what are some essentials needed? I have lots of paper already, so besides that..

I went throught the name change process a little while back, I got everything changed (Drivers license, Social security, as well as my birth certificate) I had to send in the court order with the fee to get a new birth certificate with my new name on it. BUT, where my new name is on the birth certificate.. just below the new name.. is my old name. clearly visable but it has a line through it. im wondering why?. I got the damn thing changed so I could leave my old one behind.. no more questions asked about it. it pisses me off because its a huge process to go through. and I still have this on my new documentation. I do know its now an alias, but i mean really? My mother has access to all of the screens (birth records, dmv records, etc.) and when she looked me up.. all she could find is my new name.. the old one is completely gone (kinda) I live in oregon, and im sure each state is different. Anyone know why they sent me a birth certificate with my new AND old name on it? why cant they just keep my court order and old birth certificate on file? (which they are retaining both) is there anyway to get a birth certificate with just my new name on it? im guessing a passport will give me the same problems huh?. any info will be greatly helpful, thank you.

Compute the number of sequences of letters that consist of at least one letter and at most 4 letters that can be made using the letter from the set {A,B,C}. why is the answer 120 sequences, explain and show steps how do you do it. is there some pattern?.
