My fiance legally changed his name (first and last) and this had nothing to do with getting married, it was just because he desired to do so. (He changed it around 5 or 6 years ago.) He has unfortunately lost the certified documents for this. I have looked all over the web to find out how to obtain another copy, but end up misguided. Does anyone know the actual web address of where to go to find this?.

My father is 65 and was diagnosed with organic dementia about 2 years ago. It had a rapid onset due to a severe infection and has since slowed until very recently. He is still dressing and feeding himself, driving (much to my dislike... he was driving down the wrong side of a busy road last week), and trying to operate as normal. He is a VERY strong-minded and independent person and will not take help from anyone of any kind because, in his mind, he can do it all himself and he is smarter than anyone else. (That is just his personality, even when I was growing up it was always like that.) There have been two incidents recently where he has become violently aggressive and verbally abusive towards myself over things I would consider very mundane. The first episode, about 3 weeks ago, he called my cell phone and my battery was dead, sending his call to voice mail. When I got home, he proceeded to yell at me, call me stupid, throw my phone at me and then shove me out of his room. Today, he was aggravated at me because a door in our house has trouble locking and I couldn't get it to lock. He said he would fix it for me, so when I had to lock it, I asked him to come do what he said he would do. He then yelled at me again, calling me a stupid little girl, and told me I better figure it out myself. I know it's not "him", it's the disease, but I can't cope with him anymore. His other daughter will not involve herself. He will not willingly go into assisted living which leaves me alone to care for him. I am at a point where it is endangering my health to continue this way. Does anyone have any suggestions on coping with this?.

Firstly, think about what the chef's major problems are. Its a new team that he needs to form. He is very worried that he will hire what turns out to be a bunch of lazy people who will not go the extra mile to make the team a success. Don't point this out explicitly - but offer solutions in the cover letter.. . Second, what to empathize in the cover letter. Why are you passionate about food? What inspired you during the home economics class? Has there been a time when you were on a team and really had to work hard to make the team a success? Are there other ways you can show that come h*^^ or high water you will be behind that chef in whatever happens?. . Be very clear about what you are applying for - dishwasher. Show positive energy and enthusiasm.. . I'd make the letter fairly short - the important thing is to intrigue the chef enough to get an interview.

Yes you are awfully silly I am sorry to say. You should have kept copies of the letters you sent out when you applied for jobs (you can photocopy your letters at your local newsagent for 3p per sheet).
