All you have to do is send your parents an email and you are done. Also, you could fax one to your parents with your signature.. . A release form is just a consent. All you need to do is type or write something like:. . To whom it may concern,. I hereby allow Mr and Mrs. _______ to be the legal guardians and allow them to take my son for treatment at a doctor while I am town.. - (Insert Signature here).
You mean deed poll.... not depol and you will need to go to a solicitor.
I have to write a proposal outlining my rates, compensation expectations and the like for a graphic design project. This is my first time freelancing and I'm wondering if anyone knows of a template or a sample that I can look over.. . Please help! :). Thanks!.

I have drafted a document apparently using a "pleading" template. How can I remove it? I do not want the line numbers from the template (and they are not removable as regular line numbers. I also do not want the vertical lines on the sides. I am using Word 2010.
