You can print that one, fill it out and take it to the courthouse and file it with the office of the clerk. They will give you a court date for a judge to hear your petition. It's all pretty easy to do.
You could try telling your parents what you have just said - That you love your family very much and you think King is such a beautiful last name that it seems a shame that it died out so you would like to honour your family by using it as your last name in memory of your ancestors whose line has ended.
