Honestly, when it comes to something like this, there's no real format. Write a letter with all pertinent information attached, such as his/her name, location, number, that sort of thing. Then, just go ahead and write as much or as little as you'd like. Be supportive... that's all that is pretty much asked of you.. . Your Answer:. You say you want to "change my sons last name" but it isn't your son. It sounds like the father has a court order of support, therefore the court recognizes him as the legal father. You can't just change his name. You can see an attorney who will contact the father. I understand she wants them to have the same last name, but she needs to do it legally. And remember, it is not your son! Marry her and adopt them if their father's aren't in the picture.

You go to the county courthouse where you reside, they have forms and you can do all of it yourself, after you fill out the forms take a copy of your birth certificate, 2 forms of picture ID with proof of your current name and address and it is a simple procedure.~
