I am seeking a form for my mother who is suffering from alzheimer's. Can anyone direct me towards a place that is legal for all financial uses?.

Is he wanting any type of custody shared? Are you getting child support already?

Hospital suggested health care proxy and power of attorney forms be done for my father. They set up the mental evaluation and completed the paperwork. Problem is.. I was the only sibling present when this occurred. My name was put on the paperwork and the rep from the hospital asked me if we wanted anyone elses name on the forms. We (me and my dad)both answered yes.. my brother. This was also done. I have not yet discussed this with my brother because even though his name was added I think he will be upset because he was not physically present when this happened. I probably could have told them to hold off for a few days until he could of got there, but since everything was falling in place I just said go ahead and get it done. How can I explain to him I wasnt trying to hide anything? I didnt call because I was afraid he would get mad about not being there and I really felt like this shouldnt be put on hold. Did I do something wrong? and how do I get my brother to understand? Thanks..

I am applying for a position and the employer has asked me to submit a writing sample, however, I am not sure how to format my sample (ie should I put a line or two clarifying the assignment I was asked to write about? or should I just send the email and say that I have attached the writing sample they requested and let them figure out what it's about?).
