If your father is competant, you need to discuss this with him first, since you cannot get a DNR (do not resuscitate) [not DNA] order without his consent.. If your father is not competent to make medical decisions for himself, then you need to have a durable health care power of attourney order signed in order to make decisions on his behalf.. Then you need to talk to your father's primary care physician and get the paperwork for a DNR signed (his doc may be required to co-sign) It is usually one page affair and doesn't take more than 10 min to do.. If your father is hospitalized, you need to have a copy of his DNR order on his chart and make sure that the staff is aware that it is in effect. . A DNR order is a guideline, not a law, so EMS services, if they are called, are not specifically obligated to honor it, if they are unaware of it's existance. . DNR order can encompass everything from no intubation, no CPR and no drugs and all shades in between.. Make sure all your father's doctors are aware that he has a DNR order and that they have a copy for their charts. . Good luck!

Where do I take Power of attorney forms after they're notarized?

OK, create a table with an ID(autonumber), category and date.. then create a table for your items and add to the items table, a category ID(number) field too.. create a relationship and Link the category and items table by using the categoryid field that both tables share.. now using the form wizard, create a parent /child view by adding the category and items tables to the form, so that the category table is at the top and the associated items for those categories are displayed below.. Now you can insert a category and date to the top table - and then add as many items in the lower table as you wish - they will all be linked (related) to the date and category above. when you move to the next record in the top table, then you can insert yet another category and date and repeat the process again.. Have a play with the forms wizard - if it goes wrong then delete it and try again - it's fairly easy to create a parent /child view with just two tables - quite quick too.. HTH

Cover Letter.?

I understand that many banks will not cash checks with a Power of Attorney ... that is not my question. My question is *IF* they cash a check with a POA, what do they do with the document? How does the paperwork flow?. I am going to cash the check at MY bank, the original payee of the check does not have an account there..
