I am trying to create a durable POA that also covers the medical POA - a combined document, which I've heard is possible. Is that true? Also, on every sample legal document, the 'residing at' phrase is included. Does that person's home address HAVE to be included, and if so - why? What if they move? Does the document have to be updated?. It seems that someone could be located based the Social Security Number in the document. I'm in a situation wheremy mother may need to be moved a couple times in the next year or two - because she is declining mentally. If her dementia worsens rapidly, she'll need more intensive care. She doesn't live near me, and going out of town, physically rejoining other parties included who also live in different cities, etc. is quite difficult. I was just hoping it could be avoided..

Does anyone know good tax modeling software, websites or general information?

What I am wondering.....my partner and I do not have a domestic partnership as our state does not allow this. But I am wondering, he has no family around here, if something where to happen to him to where he could not give consent for me to be able to make medical decisions, is there a form or letter than we can put together and have notarized that would give each other the rights to the decisions and information god forbid something happen to either of us? PLEASE HELP!.

Your best bet is to speak with a lawyer directly regarding this. Every province and state has different laws pertaining to will's so it's best to get good information from your lawyer. Asking the same questions you posted to your lawyer would be a good start and ask him what you should know. Most estate lawyers have done thousands of cases and will be well versed in what commonly happens to a persons assets when the person passes away.. . One thing to keep in mind is this. Two problems with a Will are: 1) They become public record. 2) they are contestable in court. This mean that eventually anyone can look at the Will and any person can make a claim that they should have been included in the Will (such as your aunt, or anyother relative). Judging by your line of questions, it sounds like there is concern about your Aunt wanting things (maybe I'm misreading the tone), so I would ask the lawyer about the process of contesting the Will if your Aunt goes that direction.

I don't understand why you need to ask for 'payslips' as this should be automatic as should be your tax form P45 and, if you leave after 5th April your P60. . I would simply say 'I would be grateful if you will send my the outstanding payslips and my Form P45 to my home address as above.'. . DON'T FORGET TO PUT YOUR ADDRESS AT THE TOP of the letter or provide an alternative.
