Hospitals always have patients sign forms indicating they are responsible for payment if their insurance doesn't cover their stay. Anyone who shows up for treatment then doesn't want or expect to pay for it should be turned away. Nothing is free.

You can get an application and fill it out and you have to mail the Original Marriage License and your Original Driver's License in...They will mail it back to you , but if you are close I would just go by the office.

I'd recommend a trust if your Dad owns a home or other assets since it avoids probate. 5) A list of assets and where and what they are. 6) List of his final wishes people to contact etc.. Damage was done to my car due to a hole in the road and the excavator said they'd reimburse me. Hasn't done that it's been a month, won't return my calls, etc. Long story.. . Anyways, I'm looking for a template I can use, basically something that states my intent to take them to small claims court. Something professional/legal looking..any ideas on where to get something like that?. . I'd just have a lawyer send one to him but I'm not trying to recover enough $ for a lawyer to get involved - gotta do it myself!. . So I just need a template so I can mail this off certified mail, hopefully he'll pay me back before it gets to small claims! Any help would be appreciated!. I have contacted a few lawyers and they have simply said go to small claims, even though I explained that all I'd need is pretty much a letter showing intent.
