Should I include a cover letter with this job application form?

Hi. I was born out of the country and was given an oriental two letter name. It's just "Sa". My last name is also "Yim" so you can understand how awkward it is when a teacher or office personnel first sees my name and says it but it sounds like they're making a breathing noise.. . I want to legally change just my first name. I'm entering college later this year so I want to fix this all up before I enter. I've been Googling and such but it's a bit confusing. I live in the Orange County of Southern California and so does anyone know how I can change my name through physical ways? I'd prefer to do this all in person and at offices and such. Just tell me where I should go to get this process started!.
