How do I make this email form?

One day my issues got hold of me and I decided to not come to work. I call my boss and let me know and he was fine with it. But then I also stayed away from work the next two days without letting my boss knows. and that triggered the termination.. . So the two main causes of poor performances were: not really happy with job and having personal issues that were affecting my performance. In the end I got fired.. . Now I am trying to apply for new jobs in the oil and gas industry and also related fields like mining and exploration but I don't know what to do. I have lost confidence in myself and I dwell on my termination. I have updated my resume but I am scared if they interviewer ask me about my old job and why I left. I don't know what to say and how to tackle this. Should I tell the truth and say I was terminated for poor performance? and if so how could I still make the interviewer believe that I still have the potential to be a good, reliable employee dispite my job termination. Should I just don't include the job in my resume?. . Any advice would be appreciated. I really need a job and I want to be able to succeed in my future job applications regardles of having been terminated for poor performance.. . Hear from you soon..
