Trust me, it's not all it's cracked up to be..

The nature of demand is given as 143(1) and the assement year as 2008-09. What should I do now. How do I contact income tax office and check whats this all about. I have my form16 and 12B forms with me. Could anyone help me out on what do to now? Thanks.

I don't know of a "checklist" anywhere. But, unfortunately, I've done this more than once because my son has my maiden name, and after my divorces (I'm embarrassed that's plural..but I digress..) I go back to sharing his last name. . I suggest starting with small things that aren't legal, if you can, before your divorce is final. (Like email accounts, phone numbers listed in the phone book, electric bills...etc.). After you have your divorce decree, which should include that you want your name to be changed back... then the more difficult stuff starts.. 1. Change your Driver's license first, because most places are going to want to see your new DL.. 2. Then the bank.. they should need your divorce decree and your new DL or ID with your name change.. 3. Then your SS#... this one is a pain.... but you'll fill out some paperwork you can usually get from your County Clerks office, or they may offer it online now, you'll have to mail them your old SS card and your original divorce decree. They mail it right back (or they do in my state) but this is why I recommend doing # 1 and 2 first.. 4. In the mean time, doctors offices, etc.. will usually just change it for you when you ask. . 5. When you get your new SS card, you then can change stuff pertaining to your employment (and taxes) Retirement, Pay checks..etc./ Note here.. if your ss # hasn't been changed by the time you file your taxes this spring, you'll need to use the one the government has on file (even if everything else is changed) or that will delay your filing and/or return.
