Use the contract with your landlord as your template. It's hard to answer this, since we don't know where you are, but your lease will have all the local laws in it. If that doesn't work, call an apartment complex and ask if you can see a copy of their standard lease. Good luck!

how does this name change thing work?

You both need to go out and get yourselves a headset / microphone and webcam. You can get both for pretty cheap (at least in the US) and then use Skype. Skype is free, and it's like making a phone call through your computer. I think you'll be amazed at the clarity of the calls.. . An online relationship is not going to be good for anything but chatting, unless you can have some kind of physical contact. Given that you live in two different countries, Skype is the best thing you have going for you.
