Not sure what you mean about the alias statement because that would not apply to you. That only applies when you are using a name other than your birth name and have not had it legally changed...this is legal but should be reported as an alias to agencies such as the IRS and the Social Security Administration.. . Now, as for the passport and any other documents that you apply for going forward they should have your new name and only your new name. You may have to provide a birth certificate in some instances to apply for certain documents like a passport but that doesn't mean they will put your old name on the document your are applying for.
