I need to determine the health care needs both medically and custodial for my mother for in home health care. I need a sample that would be used for medicare guidelines. I cannot find one anywhere. I believe it is called a patient care plan. Please help as soon as possible. I will be providing medically necessary care and working through a health provider. I am an in home health aide by trade. We will be billing her primary and I need an assessment form to determine what is medically necessary so that my boss can bill her insurance and then in turn pay me. My mom cannot afford to pay me and I cannot afford not to have an income while caring for her and I cannot physically or emotionally work outside my home and take care of her and my disabled son..

What she is asking here is that a divorce be allowed and her name is being restored. She is reserving the right (wanting to do it later) anything else that might come up. I also call parties if they are not represented by an attorney. If they are, I call their attorney.. . However, something is clearly not right with what you describe. This is a criminal matter, not civil. If you two got into a fight and she pressed changes, then the contact should be with the police, not "her attorney." Frankly, she wouldn't have one except if the charge was referred to the city or county attorney's office and someone was assigned to the case. However, that would take time - probably more than just two days.. . Honestly, the whole thing smells very fishy. What I would do is call the police or city or county attorney's office and find out what is going on. Maybe the joke is on her and what she is doing to you is a crime and she's the one who is now in trouble.. . Don't be afraid. Just get to the bottom of it.
