He was also counsel of record before the United States Court of Appeals for the Seventh Circuit in Plaintiffs' successful challenge to the district court's denial of class certification in Butler, et al. v. Sears, et al., Nos. 1:06-CV-7023, 1:07-CV-0412 & 1:08-CV-1832 (N.D. Ill.). Additionally, over the past year, he secured major victories on appeal before the Third and Eleventh Circuits.

  Jason?was heavily involved in the?In re: Imprelis Herbicide Marketing Sales Practices and Product Liability Litigation, MDL No. 2281 (E.D. Pa.), in which the Parties reached a major settlement that, if approved, will provide relief to all homeowners and business owners injured by Imprelis. His other cases on behalf of consumers include the British Airways Fuel Surcharge Litigation pending in federal court in New York, the?Checking Account Overdraft litigation?pending in federal court in Florida, the?ING Direct Litigation?pending in federal court in Delaware, and a settlement with LG over certain refrigerators?after litigation in federal court in New Jersey. He also represents Hong Leong Finance, Ltd (Singapore) in litigation against Morgan Stanley relating to the sale and marketing of certain financial products.

  Jason received his J.D. with honors from the University of Michigan Law School, where Jason was also the recipient of Michigan's highest merit-based scholarship, the Clarence M. Darrow Scholar award, and a member of the Executive Board for the Campbell Moot Court Competition. He served as a consultant assisting the Democratic National Committee during the 2004 election, helping to successfully litigate issues of voter fraud in Ohio and Pennsylvania.

  Prior to joining our firm, he served as a Judicial Law Clerk to the Honorable Kathleen M. O'Malley of the U.S. District Court for the Northern District of Ohio. I actually had a role in developing the forms you found. They are sufficient for a power of attorney IN ILLINOIS. We wrote them for people in Illinois to use based on Illinois statutes. The requirements, both as to substance and witnesses, vary by state, so if you are not in Illinois, then you should not use the form you are linking to.. . As for your second question: You do not need to take them to a courthouse. The courthouse wouldn't have any place to put them. You give a copy to the person you select as your attorney-in fact. You should also provide copies to your doctors, any hospitals you frequent, and your commanding officers.. . A 17 year-old might have some problems acting as power of attorney for many reasons (including lacking the capacity to bring suit to enforce the poa). You should pick someone who is 18 or older. You can re-do the forms to name your sister when she turns 18.

who ever told you that it changes "automatically" is dead wrong. It all starts with the social security card. Depending on where you are in ohio, you have to go to the local social security office and complete a name change request. You'll have to have a signed and sealed copy of the marriage licience, as well as photo ID. Once you recieve your new card (it'll be the same number, just with your married name on it), you can get your drivers licience changed, because they usually require two forms of ID. As for your exam, the social security office will give you a sheet of paper stating you are in the process of changing your legal name. Take it with you, and show them both your ID and the paper (It'll be stamped). You shouldn't have any problem after that.
