How do I get a court order for a name change?

I was looking for the web for an answer to this question. I have been a life-long resident of California and I recently changed my last name because of personal reasons. I have been approved by the court and have a certified copies to accommodate it. I know that I will now need to change my Social Security, DMV, and other information.

English homework... Writing a business letter (PLEASE HELP!!)?

I want to submit my resignation letter but i am not sure what to say. I read up on it a bit and most people say that you should not complain about being unfairly treated in the letter. that is not my concern though. what I am worried about is when my manager asks me why, which is going to happen.

Ok so I am the plaintiff in a personal injury case. I want to go to this renowned doctor across the country to fix my heath problems. I have not called his office or ever had any contact with him other than looking at his website. During the deposition, the defense lawyer asked what doctor I want to go to. And I said that I found one on the internet and looked at his description of surgeries on the internet. He demanded I give his name and location. So now I am worried that the defense attorney will call up the guy and somehow alienate me from my dream doctor. Am I being paranoid??? What if the dream doctor fears I am trying to bring him into a lawsuit?. What if he does something like send over my name/medical records and asks him to review whether I need the surgery...then the doctor will refuse to see me since he doesn't want to be brought into my lawsuit.. Ok but I thought I signed a release for the defense lawyer to have access to all my medical records and talked about my case/personal health/records with anyone if it pertains to the case. Am i still paranoid?.
