My biggest question is if a W-5 (EIC) form is my duty to provide the employee upon hire or their duty to provide me if they wish to receive advance EIC payments. Plus, is there anything I'm missing?.
I think that sounds great. The one thing you left out is how you'd like them to respond. If you want them to sign a release of some kind, you will need to include that release and add a paragraph asking them to sign it and return it.. . If you need a letter frrom them, then ask for that.. . In short, it looks great, but you did not tell them how to provide their approval to you.
