I changed my surname 6 months ago by deed poll and now i really need to change it back to my birth surname i was wondering if it is possible to change it back with my doctor and medical information with just my birth cirtificate and pass port and recent bank statement with name and adress on it? as i need it changed by next week i havent got enough time to change it again by deed poll :| its URGENT!!
