If she the birth has already been registered and a birth certificate issued in the 'old' name, I believe you would have to change the name formally via deed poll.

How do I write one??? Could you give an example??.

A sample formal letter?

I'm writing a fundraising letter for my intercollegiate college sports team. Any pointers?

I was convinced by my former divorce attorney to give my ex-wife Power of Attorney to sell our home. I was planning to move out of state and her attorney felt it would be difficult getting a signature from me out of town. The courts granted the Power of Attorney. She tried selling the home at a price significantly lower than we originally planned. I tried to stop the process, but her attorney told me to stay away in a nasty way. Well, several weeks later, the home is at closing and I recieve a phone call from the real estate company and they tell me that they need an original copy of the Power of Attorney to process the sell. My ex-wife and her attorney cannot produce it the original. It has somehow been lost. I've been told that the sell would not go through without this paperwork. I would only agree if the price was $10,000 higher. What rights do I now have? Can I revoke the Power of Attorney because I don't agree with the sale price? Can I negotiate a better price for the sale? I fired my last attorney, should I hire a new one? Will the courts try to step in at this point?.

I'm trying to sell my car, but I can't find my title.. I got married few years ago and I haven't changed my name.. I don't know if I should duplicate my title and request to change my name and address at the same time (I don't know if it's possible) , or just request changing name and address.. I've been searching internet for a while now, but still don't know how to do this.. Please give me advice..
