Actually the photographer owns the copyright the moment he presses the shutter release and does not need your signature.. . The signature is a release which describes what the photographer can do with those images.. . You really need 1) a copy of the release and 2) assure that the release allows you to use the images the photographer gives you for self promotion ... this includes any profile you have on Model Mayhem, One Model Place, Facebook, MySpace or any other means you use to promote yourself as a model including a "Permission to Print" letter from the photographer so you can add those prints to your book (portfolio).

No the form does not need to be filed anywhere. The person giving power of attorney has to have his/her signature witnessed by two people, or witnessed by a notary. The person/s given Power of Attorney, then retain copies of the form to show when they need to exercise their rights as PoA and perform duties in connection with it.
