What do the letters "MPH" signify, following a doctor's MD.?

My friend has her property in an irrevocable trust in her name only (widowed), with instructions that upon her death, it be trusted equally between her eight children. Further instructions state that upon their death, it be trusted to her 22 grandchildren, after that, the property is to be sold. Her question is if one of her eight children should pass away, would their spouse be taken care of before it goes to her great grandchildren? (State of Hawaii)?. Thank you..
