I basically had the same problem. My name was the same but was spelled differently -- one way on my birth certificate and another way on all records, dating back to when I started school. I don't know what state you were born in. Where I was born, I had to file an application for change of name on my birth certificate with (I think) the State Registrar's Office. Since all my records other than my birth certificate had the same name spelling I wanted it that way on my birth certificate. I believe I had to submit three documents giving the name I wanted to use. Submitted a copy of my marriage license, one of the children's birth certificates and school transcript. Actually when I knew what to do, it wasn't really such a task and only cost a few dollars. When all this was done, I was furnished a certified copy of my corrected birth certificate. Good luck.

The Hindu Marriage Act,1955 or for that reason no other Matrimonial enactment in India provides that after the marriage the woman has to change or not to change her first or last name as the case may be. The legislature no were provided for this change of name or no change of name after the marriage for the Indian women. This is a custom or usage being practiced for centuries in India & it is the Indian culture where the women of her own accord or being part of the society changes her first or last name in consultancy with her husband & other in laws. Law of the country no where ask them to change or not to change their name. If you want to check where in the Hindu Marriage Act,1955 there exist in any section,line,para with regard to change of name for married Hindu woman, then sorry you will not find it there, the Hindu Marriage Act,1955 no were provide for such provisions. If the married woman doesn?t want to change her name no one can force her to do so it?s entirely her free will to do it or not to do it.

It differs from person to person so I would not comment on particularly this physician who had multiple strokes and has Hypertension because without seeing him, it is impossible to give an opinion about his mental status and competency to make decision. It is his physician and family member who can reach to some conclusion if they feel patient is not competent. Sorta.. . I once wrote a letter to myself when I was ten and wrote down everything there was to know about me. Then I put it in an envelope and locked it up in a safe. I plan on reading it when I'm twenty and see how much I've changed in personality.. . I think maybe I'll do that again sometime, just because I'm bored. I'll write a letter now and read it when I'm twenty-four:). . So yeah, I've kinda written letters to myself...... well, to my future self.
