i have no idea how to write one and i need help please.

I officially changed my birth first name til I was19 only because people keep mispronounced it and its very annoying. . My family and close friends still called me by my old name and I preferred it that way too. I even ask my husband to call me by my old name even though I met him after I changed my first name. I took my husbands last name and also kept my madam name as well.
