You have the "action" on the form call up a server-side CGI script, which can read the submitted variables and do various interesting things with them. This can be in any language but Perl, Python and PHP are the most popular.. . That being said, I've written CGI scripts in Bash. But more as proof-of-concept than anything serious.

If you aren't going to add a resume, put the cover letter underneath the application form. Employers will likely sort the application forms if they get a lot of them, and if your cover letter is covering it, they may be an annoyance or a hindrance -- something you wish to avoid.
