How would I fill out a change of address form First and Last name with the name Fusion sign and design?

    resignation letter?

    .

my g/f changed her name back to her maiden name after her divorce and done it on the net. only cost ?15.. . i think you have to have it certified by someone though, like a solicitor or something, someone else may know this?

So my uncle is Ina nursing home and I've had power of attorney over him for years. His health is really bad lately, due to a stroke. Recently I visited him in the nursing home and they told me that they changed his power of attorney over to his step son and that my uncle signed for it, yet he doesn't remember signing for it. I thought that if a person had a power if attorney that they couldn't make changes like that without me signing away my rights?.

I have disputed a utility account for a measly $106 that is not mine. The thing is that it is in a bordering state that I have never lived in. I proved I did not live there and sent police report that was filed for ID theft, but they still won't remove it. This was done last year and I thought it was done until I got my new report. I called them and they still said i was responsible. I know it is not much but ITS NOT MINE and I am Not paying it!. I didn't know about DV until recently when I came on here. That is my next step so any suggestions are appreciated. . Oh, and does the CA have to be licensed in my state or the other state?. Thanks Summerly. I did contact the OC and they are saying it's mine, but the CA is the only one reporting it so i thought I'd go that route. What do you think?.
